1. I think it's just a long running joke with no punchline to leave the audience guessing… I don't think there's anything out of the ordinary meant to be under his hat… And the comments from the other characters when we believe his hat is off is a misdirect to make us think that there is something wrong… It's kind of like Boomhauer from King of the Hill… We had no idea what he did for work until the very last episode where it was revealed in his wallet… The creators wrapped up this show without putting this conspiracy to rest which makes me believe that if we were to ask them personally they wouldn't have an answer.

2. Ed being, um… kinda dumb. He might have thought that Edds hat was attached to his head.

Well…judging by the animation of it, I’d say that the “It’s gym class all over again!” is from the original Ed, Edd n Eddy seasons where the trio and the rest of the cul-de-sac have sort of an “endless summer” thing which “Phineas and Ferb” took when it first came out; but the other parts are of the ‘going to middle school’ thing going on there, for the ‘endless summer floating timeline’ goin’ on there. No? 🤔

I know this videos is kinda old (2017) but I came with my own theory while watching this and I really think it's more simple and it has to be with him being alone/without parents at home. cuz, you know, no one is taking care of that and considering how unfashionable his style is, I think he might have cut his hair himself every now and then and just mess it up a bit so he has no choice but to hide it. And this could be something that has happend before he knew ed and eddy cuz, you know, he's alone since he was a child… kind of why I think why ed had that reaction too, and sarah was kind of fangirling over edd, so maybe she thought it was perfect anyway… (?)

I think that edd had cancer and his hat/beanie is the only thing that's keeps others for knowing his past but for the 3 strains of hair from the back of his head I think the cancer stopped spreading but still he's still gonna think that people will ashamed or probley sorry for him but the only ones that know this is ed and eddy that's my theory

    Edd where's a hat for the same reason as to why I where a hoodie. To subconsciously hide a mental traumatic brain injury.

    He may have hit a wall, I hit concrete.

    No scars; but mentally there's a cut
